Engine failure and poor maintenance caused Tema Aircraft crash – AIB Ghana

The Aircraft Accident and Incident Investigation and Prevention Bureau (AIB Ghana), tasked with investigating the cause of the microlight aircraft crash in Tema in March 2026, has presented its findings, citing poor maintenance and engine failure.

The accident, which occurred on March 16, 2026, led to the deaths of two siblings: Captain Frank Amoaning Donkor, 36, a flight instructor, and 25-year-old Elijah Ofori Donkor.

The accident aircraft, with registration number 9G-ADV, was flying from Ho to Accra when it crashed within the premises of the TMA Day Care Centre before catching fire.

The Bureau, presenting the final report on Tuesday, May 26, the head of investigation, Capt. Paul Forjoe explained that there were poor professional maintenance practices and engine failure.

It noted this follows repeated overheating incidents during short flights and landings in Ho involving the aircraft.

However, the report added that the situations were not properly addressed before subsequent flights, emphasizing that an overheating-induced engine failure led to the crash.

“The investigations believe that the aircraft most likely had an engine failure due to overheating,” AIB Ghana said.
